Description

Fire

Section required for Incident Types 111-112, 120-123, 130-138, 140-143, 160-163, and 170-173.

The Wildland Fire section can be used instead of the Fire section for Incident Types 140-143, 170-173, and 632.

Structure Fire

Section required for "Incident Types" 111-112, and 120-123.

Civilian Fire Casualty

Section required if casualties indicated in Response section for Incident Types 100-199.

Fire Service Casualty

Section required if casualties indicated in Response section for Incident Types 100-199.

EMS/Search & Rescue

Section can be completed for "Incident Types" 311, 321, 322, or 323 or when an injury is treated in any other incident type.

HazMat

For one of the following.

  • Specialized HazMat resources were dispatched or used or should have been dispatched or used, for assessing, mitigating, or managing the situation,
  • Releases or spills of hazardous materials that exceed 55 gallons.

Arson/Investigation

This section can be used whenever the Cause of Ignition is “intentional,” or “under investigation” without any distinction made as to whether or not a crime has occurred, or a determination of criminal intent. This section can also be used when the fire is under investigation or in cases where the cause is "Undetermined after investigation.”

Add vehicle and extrication information when applicable.

 

NFPA 1710/1720 Information

NFPA 1710 (for career firefighters and EMS personnel) and NFPA 1720 (for volunteers) define minimum response times to an emergency and minimum fire company and EMS staffing levels. The NFPA 1710/1720 Information form provides a method for tracking compliance with the standard.
