Illinois EMS Incident Reporting
FH is used in Illinois to collect EMS incident reports. This functionality was added to FH version 5.3. The steps to install the Illinois EMS functionality was included in the 'Implementing Illinois State EMS Reporting' instruction.
Note: FDID is required. If you are EMS only, use your EMS service number for FDID.
Patient/Victim Information Form Completion Guidelines:

"No", "Unk", and "N/A" are valid entries for Driver's License # and Social Security No.

Aid Given defaults to Yes.

Signs & Symptoms are required. These are the signs and symptoms of the Chief Complaint.

If Provider Impression is "16", a Cardiac Detail record is required, "38" makes an Injury/Illness record is required. Alert Criteria in the Disposition & Transport section is required for both "16" and "38".
Past Medical History defaults to Yes.

If Treatment Authorization is "2", a System Resource Hospital is required.
Vitals record numeric fields (Pulse, Resp, BPSys, BPDia) allow "777" as a valid entry for no pulse, no breath, and/or no blood pressure.
You must select Yes/No/Unk/NA for the Patient Staff Exposures field.
You must select Yes/No/Unk/NA for the Exposure Precautions field.

One Report Authorization Record is required to gather the EMS Certification of the highest ranking EMS provider for this patient.
