View Advance Beneficiary Notice (ABN) in an incident

An Advance Beneficiary Notice (ABN), also known as a "waiver of liability," is a notice that suppliers and other medical providers are required to give a patient when they offer services or items that they know or have reason to believe Medicare will determine to be medically unnecessary for the patient, and therefore, will not pay for.

  1. Find the incident you want to view the ABN for, as described in Find an incident by date, or in Search for an incident.

    In the lower left corner of the page, a Results section appears, listing all of the incidents that match the search criteria you defined.

  2. (If necessary) Click a column name in the title bar to sort the contents of the column from ascending to descending order, or back again.
  3. Do one of the following.

  4. Under Forms, click ABN.

    Note: The ABN icon appears on the QA/QI Notes for Incident Number page, and is activated on the Incident Details page, only if the medic filled out an ABN for the incident.

    The information in the Patient Care Report section of the page updates to display the ABN from the incident.
